To enroll, new parents must first fill out each of the enrollment forms below for each child. Then, contact us via email at info@sunnywest.org or calling us at (323) 643-4626 to set up an enrollment appointment with our center’s Director. The Director will determine the needs of your child, explain and answer any questions you may have about our programs, enrollment forms, and policies.
Please note that your child cannot begin attending SWCDC without a current signed Immunization Record, TB Test Clearance and the completed Emergency Card.
